ABOUT THE CONFERENCE

The Hospital Transformation Program’s (HTP) annual Learning Symposium conference is about sharing experiences, inspiring innovative thinking, and planting seeds for growth. The two-day HTP learning symposium will again provide in-person and virtual options for participants to celebrate their collective successes in HTP and provide tools and learning experiences for future success under the HTP. The HTP is now in its third program year and we are starting to see the fruits of the labor as the interventions are in their final stages of planning and implementation and are starting to impact patient care. Our community engagements and relationships are still growing and starting to mature. This year's Learning Symposium will focus on the program and community capacity opportunities that are being opened up as a result of the program, sharing our stories of impact, and ringing the recognition bell. The symposium will include presentations centered around social needs data for patient care and population health, community capacity building, data and interoperability to improve patient care, and a keynote about putting inspiration and energy into clinical transformation as part of planning for the future of quality care.

Other sessions will include following the data from Health Information Exchanges (HIEs) to Regional Accountable Entities (RAEs) and to the future of data exchange in the state, racial equity in health care learning collaboratives, and the story of us, which focuses on the amazing work within the HTP. Another keynote at the Learning Symposium will focus on Journey Mapping to a more integrated future. We are excited to showcase all of our amazing presenters at the symposium who represent both urban and rural hospitals, national equity leaders, and numerous HTP collaborators paving the way for quality hospital care for all Coloradans. To learn more about the HTP Learning symposium and our presenters visit the agenda below.
